Lost/Damaged Passports

Q: What is the definition of damaged passport? A: Damaged passport is classified further based on the extent of damage, i.e.: Damaged Passport - Passport number is readable, name is legible and photo is intact Damaged beyond recognition Q: What can I do if I have lost my passport and have to travel to India urgently due to an emergency like serious illness or death in the family? A: If you have lost your passport during traveling, then you can come to India on the basis of "Emergency Certificate". For the issuance of "Emergency Certificate", you need to contact the respective mission/post of that country. Q: What are my responsibilities when applying for a passport, if my old passport has been lost or damaged? A: Loss of passport should be immediately reported to the nearest Police Station and to the Passport Office (PO) or Indian Mission, if abroad. Also apply for "Re-issue" of passport at your nearest Passport Seva Kendra (PSK) and submit the required documents along with your passport application form. A: To apply for duplicate passport in case of lost or damaged passport, you have to apply for a "Reissue" of passport. Q: I am an Indian living in Germany. I lost my Indian passport but I don't have photocopy of the same. What should I do? A: Photocopy of the old passport is not a mandatory document to be submitted in case of lost/damaged/stolen passport. It should be produced, if it is available. But you will have to produce, the previous passport details like passport number, Date of issue, Date of expiry and place of issue, while filling the passport application form. If you do not have that information, then contact the respective mission/post of that country. Q: My passport booklet is damaged. I want to go abroad on urgent basis. What should I do? A: In case your passport has not been damaged beyond recognition, i.e. passport number is readable, name is legible and photo is intact, then you can apply for reissue of passport under the Tatkaal scheme. But, if passport has been damaged beyond recognition, then you cannot apply under the Tatkaal scheme. In that case, you need to visit Passport Officer (PO) at your nearest Passport Seva Kendra (PSK) to get the passport on urgent basis. Q: I lost my passport in the U.S. and have been sent to India on Emergency Certificate. Can I be issued a new passport? If yes, what all extra documents do I need to submit to get a passport? A: In case of lost passport, you need to apply for reissue of passport. Following documents must be submitted along with the application: Proof of Present Address Proof of Date of Birth Affidavit stating how and where the passport got lost/damaged (Annexure 'L') Police report in original Self-attested photocopy of first two and last two pages, including ECR/Non-ECR page of old passport, if available.

Name Change in Passport
It is often happened with the women that they require change of name in passport as they get married, re-married or divorce. If anybody requires change of name in the passport they should submit all the documents required for it. In general, the marriage certificate is enough. If any person  has changed his/her name without any reason then they should submit an affidavit for it.
Addition of Spouse Name
Before marriage if you are applying for an Indian Passport then while filling the application form you need to write Not Applicable in the column where Spouse Name is asked. Then a passport with out your spouse name is issued. After marriage if you want to add your spouse name in the passport then you need to apply for reissue of passport with addition of your spouse name. You need to submit your current passport along with the proof of marriage such as marriage certificate issued by registrar of marriage. Then a new passport with your spouse name is issued to you.
Deletion of Spouse Name
If you have added your spouse name in your passport after your marriage then it might be required for you to delete your spouse name if you are divorced or it may be required to you to change the name of your spouse if you are re-married. To delete or change the name of your spouse you need to submit an application form for reissue of passport with name deletion or change of name along with the documentary proofs for divorce or re-marriage or any other cause. Then after verification of the proofs the passport authority will issue you a new passport with deletion or change of name.
Documents Required
The following documents are required for change of name after marriage in passport,

• A women applying for change of name after marriage should submit Husband’s passport if any, marriage certificate issued by registrar of marriage.
• Divorcees applying for change of name or deletion of spouse name should submit certificate copy of Divorce decree, dead poll or affidavit.
• Remarried applicants should submit divorce deed or death certificate for the first spouse and marriage certificate for second spouse.

How to Apply
To apply for a change of name after marriage you need to submit an application form (Form –II) along with affidavit and documentary proofs in the nearest passport office.
Fees Applicable
The change of name after marriage comes under miscellaneous services. For this kind of services they will around 300 to 500 rupees.

Birth certificate issued by a Municipal Authority or district office of 

the Registrar of Births & Deaths; 

Date of birth certificate from the school last attended by the

applicant or any other recognized educational institution; or an
